Scheduling an Appointment
A referral from a health care provider is not necessary to schedule an appointment.  Parents are welcome to contact Dr. Akdag directly to request assessment or consultation services for their child.  

Inquiries and referrals from physicians, allied and mental health professionals, educational facilities, and community agencies are also accepted.  Referral forms are available for download (referral form).  Please fax completed referral forms to 604-648-8464. 

All communication is kept strictly confidential.

Prior to Your Appointment
After an appointment has been scheduled with Dr. Akdag, she will send out a package of materials that includes information on the date(s), time, and location of the appointment, as well as several questionnaires and consent forms for parents to complete.  You also may be asked to pass along questionnaires to your child's teacher.  Please bring the completed paperwork with you to your appointment or arrange to mail or fax the materials to Dr. Akdag prior to the appointment day.  

What to Tell Your Child
Your child may be curious about his or her appointment with Dr. Akdag.  Please let your child know that it will not be a typical doctor's visit.  Dr. Akdag will be asking him or her to do activities that help us understand more about his or her thinking, learning, and remembering abilities.  Your child may be asked to assemble puzzles, draw pictures, tell stories, or similar types of activities.  Most children find these activities fun.  Appointments for assessments are generally scheduled over 2 days, with each appointment lasting approximately 3 hours, including time for rest breaks.
